If you have witnessed an incident or have a concern that involves the immediate safety of another individual please contact emergency services immediately. If you are on campus and the incident is occurring on campus, please contact Campus Police and Security Services at 216-987-4911 (emergencies) or 216-987-4325 (non-emergencies). If you are on campus and the incident is occurring off campus, please also contact Campus Police and Security Services. If you are off campus, please dial 911.
Title IX/Sexual Misconduct Reporting Form
This form can be utilized by students, employees or others wishing to file a report on their own behalf or by anyone wishing to file a report on behalf of an impacted party. This report documents incidents or alleged violations of harassment and discrimination based on sex or gender, including sexual harassment, sexual violence and other forms of sexual misconduct. Title IX of the Education Amendments of 1972 is a federal civil rights law that protects individuals from discrimination based on sex and gender, including sexual harassment, in educational programs or activities that receive federal financial assistance.
The Title IX Coordinator located within the Office of Institutional Equity (OIE) is responsible for coordinating the resolution of allegations. Upon receipt of this form the Title IX Coordinator will contact you to discuss your complaint, offer supportive measures and discuss the College’s procedure to resolve the complaint formally or informally.
